Document Submission Guidelines

Submit documents in PDF format wherever possible. Scanned copies must be legible and complete.
Include all Bed-Head Tickets (BHT), discharge summaries, operation notes, investigation reports, and consent forms relevant to the matter.
Do not submit original documents. Retain originals with yourself or your legal counsel.
Clearly indicate the treating institution, treating doctor(s), and the nature of the dispute or claim.
Submissions that are incomplete, illegible, or lack mandatory disclosures will not be processed and fees will not be refunded.
